The annual information, transportation, and residency verification forms are now open in the PowerSchool Parent Portal.
PLEASE NOTE these forms will ONLY BE OPEN UNTIL JUNE 1.
IF THESE FORMS ARE NOT COMPLETED, PowerSchool access will be locked for those families and students and will only be opened upon the completion of these forms.
ALL FAMILIES should complete for each student:
1. Annual Update of Student Information Form
2. Transportation Request Form
3. Annual Health Information Update
4. Residency Verification:
Grades 2, 5, and 8 should complete the Annual School Change Verification (required by the BOE when changing schools).
ALL OTHER GRADES should complete the Annual Residency Affirmation and Intent Statement.
